Immunizations and other requirements


Requirements to be completed after you are accepted into the nursing program and before August 1st 

  1. Evidence of either a negative TB skin test within the last year or negative chest x-ray within three years.
  2. Documentation of a completed series of Hepatitis-B vaccine, documentation of having begun the series, or documentation of informed refusal of the vaccine.  The series consists of three immunizations.  The initial vaccine, a dose 30 days later and the final dose 6 months later. See Hepatitis B Consent form 
  3. Documentation of varicella (chicken pox) vaccine, positive titer (blood test which tests for evidence of the disease) or documentation of informed refusal of the vaccine. Two doses of the vaccine are given 4-8 weeks apart.  See Varicella (Chicken Pox) Consent Form
  4. Documentation of two Measle, Mumps and Rubella (MMR) vaccine.

We do accept copies of your official immunization record.  They may be faxed to 731-983-3016, scanned and emailed to emanderson@fhu.edu or mailed to Nursing Department, Freed-Hardeman University, 158 East Main Street, Henderson, TN 38340

Yearly requirements are completed near the end of each fall semester.

  1. Evidence of negative drug screen.
  2. Evidence of professional liability coverage in the amounts of $1 million per claim/$3 million annual aggregate. Two websites are:  http://www.proliability.com/ahc/http://www.nso.com/pli/index.php, but you are free to purchase it elsewhere.
  3. Evidence of a negative background screen.  Sixty (60) days before the January clinical you must have a background check.  The cost will be approximately $65. You will apply on-line using a credit card
