Freed-Hardeman University has a secondary insurance policy that overs every intercollegiate athelte who is injured while participating in their specific sport. Pre-existing injuries, illnesses and prescriptions are not covered by our secondary policy. Our insurance requires that every claim be filed first with the athlete's primary insurance. Our policy has a $250.00 deductible and a $25,000.00 medical maximum per claim. The Athletic Trainer must be made aware of the accident prior to any medical services being rendered. The treatment must begin within 90 days of the accident. Filing with the primary insurance is the responsibility of the athlete and/or the parents. The primary insurance must be valid in the state of Tennesee and be authorized for treatment at Sports, Orthopedic, and Spine and at Jackson-Madison County General Hospital.

Please return all forms along with front-and-back copies of the Athlete's Driver's License and Insurance Cards. Along with the insurance card the policy holder must include their full name, date of birth, Social Security Number, and date of effective coverage. All information must be turned in prior to the time of the athlete's physical. If paperwork is not turned in, the athlete will not be allowed to practice. All physicals will be obtained through Sports, Orthopedic, and Spine.

All questions should be referred to the Athletic Trainer or the Athletic Director.
